In order to process your request, the email address in your WinProxy setting must be used. If you are not sure what email address was registered with WinProxy, please go to the WinProxy machine and select File/Set New Serial Number.

If the address listed in your setting is not current, please update the email address with a valid email address and wait one hour before completing this form.
